Home Visit + Strategy & Support Package

Ideal for added personalized support, safety concerns or complex situations for those living in the lower mainland of BC Canada.   

$460* | Up to 3 hours

• Up to 30-minute initial call with a Client Care Coordinator to gather information in preparation for the home visit.
• Up to 1.5-hour home visit with Karen Tyrell, CPCA, CDCP Dementia Care Specialist. This visit includes observation, assessment and verbal recommendations.
• Up to 15 minutes virtual check-in with a Client Care Coordinator.
• Up to 45-minute follow up call with Dementia Care Specialist, Karen Tyrell, to discuss recommendations’ implementation. Can be used for a facilitated family and caregivers’ meeting.
• 2-month access to the Dementia Solutions – Family Support Membership – a subscription-based platform. https://dementiasolutions.ca/family-membership/

Notes:

• A Letter of Engagement will need to be signed prior to working together.
• Package hours to be used within 2 months.
• Support can be done via email, phone, text, and/or online video conferencing (Zoom).
• Other family members, professional caregivers and/or family friends can be included.
• Time is tracked and deducted in 10-minute increments as services are used.
• A Caregiver Essentials E-Booklet provided.
• Detailed formal written reports can be provided for an additional charge of $200 per report.
• Prepayment is accepted by e-transfer, via a secured online link, or via a credit card over the phone.
• No refunds for unused package time.
• Rates exclude applicable taxes and are subject to change without notice.
